NSHA MM-SR-005 “Do Not Use” Abbreviations, Symbols and Dose Designations


The use of some abbreviations, symbols and dose designations has been identified as an underlying cause of serious, even fatal medication errors.  The Institute for Safe Medication Practices (ISMP) Canada has developed a list of dangerous abbreviations adapted from the ISMP (US) list, called “The Do Not Use List.”

This policy outlines the management of high alert medications and actions required to promote the safe storage, labeling, prescribing, preparation, dispensing, administration and documentation of high alert medications.
